New File Creation Forms

If you would like to request a new file to be created, please use the appropriate form from the list below. If you are unsure of which file type you need please contact Records Management Services.

New General File Application - Use to request up to 5 files

Bulk File Requests spreadsheet - General Files - Use to request more than 5 files at once

New Personnel File Application - Use to request a single Personnel File

New Research Administration File Application - Use to request up to 5 files

Bulk File Requests Spreadsheet - Research Administration Files - Use to request more than 5 files at once

New Volume Request - Use to request subsequent volumes of any existing TRIM file (General, Personnel or Research Administration)


University Archives - Records Transfer Form

Please complete and attach the Records Transfer form with all transfers of  records to the University Archives. Note: There are some conditions that must first be met. For more information please refer to ARMS Advice Sheet 35 - Transfer of Records to University Archives.

University Archives - Records Destruction Certificate

The Records Destruction Certificate is a form that must be used by university staff to document the lawful destruction of University records within the Faculty, School, Unit or Administration Area. Note: University records must not be destroyed until approval is granted by the University Archivist as delegate for the Vice-Chancellor
